*Submission of e-mail address will result in notifications four days before items are due, when they are overdue, and when items placed on hold are available for pick-up.
If all information submitted does not match existing registration record, change(s) will not be made. Name changes must be requested in person. Upon submission of this form to change your address, you will not be able to renew items until a photo ID with current address is presented to library staff on your next visit. Use of the form to add or change an e-mail address will not require a library visit.
