To receive hold notices by text, please provide your cell phone number and provider. If you do not want text messages, please provide a contact phone number, no provider. You may be charged for text messages, depending on your cell phone service plan.

All messages will be sent assuming a MMS-enabled phone. MMS (Multimedia Messaging Service) sends html-enhanced text with no character limit. If your phone is not MMS-enabled, you may still receive the text, but the text may not be formatted correctly, may be broken up into more than one text message, or may be cut off at the 160-character limit.


If all information submitted does not match existing registration record, change(s) will not be made. Name changes must be requested in person.

Upon submission of this form to change your address, you will not be able to renew items until a photo ID with current address is presented to library staff on your next visit. Use of the form to add or change an e-mail address, or to sign up for text alerts, will not require a library visit.
